1. Acceptance and Authority

2. Services and Fees
Services Provided: My Debt Navigator (“MDN”) provides confidential, unbiased debt consultations and guidance to Employer’s employees (“Employees”).
No Cost to Employer: Employer will not pay or receive any fee for MDN’s inclusion on its benefits platform.
No Guarantee: MDN does not guarantee any specific result or outcome. All recommendations may vary based on each Employee’s individual circumstances.
3. Brand and Website Usage
If Employer chooses, Employer grants MDN a non-exclusive right to use Employer’s name and logo on a dedicated page or website for Employees.
MDN will use Employer’s branding only for identifying this collaboration and directing Employees to MDN’s services.
4. Confidentiality and Privacy
MDN’s Commitment: Employee information shared with MDN is kept confidential to the extent permitted by law.
Mutual Confidentiality: Both Parties will protect each other’s nonpublic or proprietary information.
5. Right to Refuse or Cancel
MDN may refuse service to any individual Employee at its sole discretion.
Either Party may end this arrangement at any time, without incurring liability.
6. Communications
Employer acknowledges MDN may use SMS, email, telephone, or other communication methods to provide and support MDN services for Employees.
7. No Liability and Indemnification
No Liability: MDN is not liable for losses or damages arising from Employees’ use (or non-use) of MDN’s services.
Indemnification: Each Party agrees to indemnify and hold the other harmless from claims arising out of or related to a breach of these Terms or misuse of each other’s confidential information or branding.
8. Governing Law and Entire Agreement
All communication, agreements, and services are strictly between the client and the service provider. My Debt Navigator does not intervene or mediate in disputes between clients and debt solutions providers. Any legal disputes related to such services will be governed by the laws of the State of Wyoming, and any legal actions must be filed in the appropriate courts located in Sheridan County, Wyoming.
9. Authorization and Co-Branded Communications
By submitting this form, you confirm that you are authorized to represent the company named above and consent to the use of the Company’s name, logo, and provided information for co-branded communications related to the employee benefit program. You also affirm that you are authorized to enter into binding agreements on behalf of the Company and that the provided information is accurate.
